Community Care Partners (CCP), a partnership between Bay Cove and Vinfen, is a collaboration of four community-based organizations that partner with the complex managed care teams of our ACO partners to promote integrated care for MassHealth Members. Our Care Coordinators work side by side with other healthcare providers, serving as an expert in community-based social services and a bridge to primary care., serving as the community outreach arm of the PCP office.

As a Community Partner program funded by MassHealth, we partner with MassHealth Members to improve their access to healthcare through identifying and coordinating a wide range of the services and supports they need. As both a BH and LTSS CP, we are a collaboration of four long standing, community-based organizations — Bay Cove,  Justice Resource Institute, Mystic Valley Elder Services, and Vinfen. Our multidisciplinary care teams are comprised of Licensed Social Workers, Community Health Workers, Registered Nurses (RNs), and Recovery Coaches. Find out more »

Our LTSS CP serves MassHealth Members in the greater Boston area, as well as Malden. Our BH CP serves MassHealth Members in eastern Massachusetts from Lowell and Lawrence to Fall River and Cape Cod. Our expansive service area promotes partnerships with 14 ACOs and their primary care practices. Find out more »

Eligibility for CCP is determined by MassHealth’s Community Partners program eligibility guidelines. All members must have MassHealth (Medicaid) insurance coverage and any member not enrolled in DMH’s ACCS program must also be enrolled in a Medicaid ACO plan. Find out more »

Consumer Advisory Board

The Consumer Advisory Board (CAB) is comprised of a diverse group of engaged Members. The CAB's goal is to advise Community Care Partners regarding various components of the BH and LTSS CP program services, reinforce the positive impact the program is having, and offer recommendations for any program or process improvements. The CAB platform gives Members the opportunity to use their lived-experience knowledge of health care services to contribute to the development and improvements of CP best practices.
