Community Care Partners eligibility criteria follow the MassHealth guidelines for members enrolling in the community partner program. Below is a list of criteria broken down by program (Behavioral Health (BH) CP and Long-Term Services and Supports (LTSS) CP). Please contact us for any detailed questions related to eligibility!
BH CP
- MassHealth (Medicaid) only members — Members who also have Medicare are NOT eligible
- Ages 18 to 64
- Members with:
- Mental health diagnoses
- Substance use disorder
- Complex medical needs
- Multiple emergency visits and hospital stays
- Currently in an ACO/MCO plan under MassHealth
- MassHealth members in DMH’s ACCS program can be in BH CP if they are over 18 and live within our service area; None of the other eligibility requirements apply for these members
LTSS CP
- MassHealth (Medicaid) only members — Members who also have Medicare are NOT eligible
- Ages 3 to 64
- Members with complex and/ or chronic medical conditions, such as:
- Autism
- Physical disabilities
- Acquired brain injuries
- Intellectual and developmental disabilities
- Heart and lung conditions that limit daily functioning
- Currently in an ACO/MCO plan under MassHealth
- MassHealth members in DMH’s ACCS program can be in LTSS CP if they are over 18 and live within our service area; None of the other eligibility requirements apply for these members
